Individualized School Care Plan Template
A student with type 1 diabetes needs one plan that the nurse, the classroom teacher, the coach, the bus driver, and the family all work from. This printable template walks your team through it: medical orders and the DMMP, daily routines, low and high glucose response, activities and special circumstances, staff training, communication, and device troubleshooting — with a signature block for the team review.
Fill it in with the parent/guardian, the school nurse, and the 504 coordinator, attach the provider-signed DMMP, and review it at least once a year. Print it blank to complete by hand, or — if your school uses T1D Ready™ — fill it in online and keep it with your child's records.

Nurses and district admins can complete the same plan inside T1D Ready™, where it stays attached to the student alongside the care log, CGM data, staff training records, and parent messaging.

- Start with the DMMP. Complete the plan with the parent/guardian and the provider-signed DMMP in hand, so every clinical step points back to it.
- Name people, not roles. Every line about who treats, who covers, and who is trained should carry a real staff name and the date they were trained.
- Go through it with the adults on duty. Classroom teacher, PE teacher, coach, and transportation staff each confirm they are informed and know where supplies live.
- Review on a schedule. Set the next review date, and update the plan whenever care, technology, or the schedule changes.
Stop managing this plan in a binder
T1D Ready™ keeps the care plan, CGM readings, care log, staff training records with expiry dates, and parent messaging together for each student — with role-based access, so every adult sees only what they need and every review is documented.
